Model notes4-page CVI. Page 1 lists animals 1-7 (BE breed abbreviated as Beagle). Pages 2-4 are continuation sheets. The ditto/vertical-line rule was extensively applied: LXI-3 on page 1 is the first animal with explicit Sex=F, Age=7 Wks; the vertical line runs through rows 2-7 on page 1. Page 2 starts with animal 7 (QZK-3) re-labeled as animal 7 in the continuation header, carrying F/7Wks forward through SII-3 (animal 21 on page 2). Page 3 starts with SJI-3 as animal 7 (continuation), SKI-3 has a downward tick on Sex column suggesting F continuing; LUJ-3 (animal 9 on page 3) shows explicit M sex. Males continue through SBJ-3 (animal 21 on page 3). Page 4 shows SCJ-3 (animal 7), SDJ-3 (animal 8), SGJ-3 (animal 9) with ditto marks carrying M/7Wks. Total unique animals counted: 40 as stated on form. The 'ANIMALS FOR RESEARCH' text appears in the Rabies Vaccine Producer and Serial Number columns — this is the stated purpose of the shipment. The Parvo Virus vaccination (NEOPAR, 06/29/23) is the substantive vaccination. Note: page 2 lists animal 7 as QZK-3 which matches page 1 animal 7 (QYK-3 vs QZK-3 — page 1 row 7 appears to be QYK-3 while page 2 row 7 is labeled QZK-3; these are treated as separate animals with the page break continuing the count). The mailing address on the owner line differs from the street address (P.O. Box 318 Mt. Horeb vs 10489 W. Blue Mounds Road, Blue Mounds) — both belong to Ridglan Farms.
